The Transitional Placement Program (TPP) was created to support East Side Union High School District, junior and senior students who are currently diagnosed with mental health disability or who are diagnosed with an Autism spectrum disorder.
TPP is an innovative partnership with Department of Rehabilitation (DOR) and East Side Union High School District (ESUHSD) that provides support to students, and enables them to successfully transform from high school to the working world.
The Santa Clara County Mental Health Department Community Services and Supports Plan identified transitional youth with mental health disabilities as one of the most at risk populations, and one of the least served. Since adult mental health services start at the age of 25 this is a critical time to engage and provide guidance and support augmenting the few services available to transitional youth with mental health disabilities. This program guides students to complete an educational program, participate in vocational planning, transition to work, and transition to adult mental health service.
Individualized employment preparation, life management, job placement and support services. By using the framework of a paying job and career development, the students learn the skills and develop the desire to create a healthy lifestyle which will support their future goals.
